Social and Community Referral Platform Social and Community Referral Platform If you are a not-for-profit organisation this is your one-stop-shop to promote your activities, accept referrals from individuals, health care workers, key workers etc. It will also link you to information on other services, not just borough-wide but services in neighbouring boroughs. Social and Community Referral Platform provides a host of benefits: Promotes the range of community services and activities in Greenwich Search tools to find services based upon health condition, support need, or location Includes a Quality Assurance framework to provide reassurance for people accessing services (and referrers, in the future) Deals with direct referrals Can be used by health and social care teams to support their work with residents i.e. Adult Social Care, health professionals, community health Ability to upload and manage activities via an online account Builds evidence of demand for services and activities to support a business case for future funding ________________________________________________________________________________________________________ Here is some more information below about the platform and what it can offer your organisation: This platform will promote both free and costed activities that your organisation/ project are delivering Activities could be advising session, play session, training activities, walks, guided tours, coffee mornings, art sessions, lunch clubs, workshops etc….whatever activities you are providing that local people can attend or engage with….. The platform will take direct referrals from borough resident who are interested in attending your services or it will provide your contact details for borough residents to contact you. You are in the driving seat Once you have created an account you can then manage your own services on the platform. You can promote new activities, you can indicate whether you are open for referrals and also indicate whether you would like individuals to Self-Refer directly into your services. You can also take down or put on time limited activities. What if I have certain criteria to accept referrals? You can indicate this on your activity listing, so anyone referring in can see the pre-requisites, criteria for you to be able to accept referrals. You can also decline referrals through the platform and inform the referrer the activity isn’t suitable for them What do I get? Your own personalised confidential dashboard to pick up and manage incoming referrals Reports and stats on the number of referrals into your services Access to a Full Training Video, once you create an account which provides you with a full overview of the Dashboard and how to use it. Once you have created an account you can Log-in and advertise the services and activities you deliver on our service directory Upload and manage your activity listings Receive new referrals into your services from members of the public and link workers Your own personalised dashboard to manage ad track all your referrals Take a look at the platform Simply Connect Greenwich - connecting you to your local community Stage 2 This will be happening in the next couple of weeks where the public will be able to self-refer themselves or a family member or friend directly to your activities. To be able to manage the referrals as per your Service Level Agreement you will need to either watch the 7 minute video which will be in your dashboard at the bottom left of the page or alternatively attend a 30 minute information session that Volunteer Centre Greenwich will be holding on Thursday 29th August at 1:30pm online. Stage 3 This is when you will receive referrals from key workers, link workers and Health Care providers. We will notify you when this stage is available.
